Forms
On this page you will find the forms relating to the disciplinary process and Tribunals.
The first form is the Request for a Tribunal Hearing. This form should be used if you have received a Disciplinary Infringement Notice for an act of Misconduct (normally this involves having been sent off during a Match).
The next form is the Request for an Appeal Hearing. This form should be used if you have already attended a Tribunal Hearing but were not satisfied about the finding resulting from the hearing. Do not use this form unless you have already attended a Tribunal Hearing.
The last form is the Request for a Grievance Hearing. This form should be used if you wish the Tribunal to hear a Grievance. For example, if your club has been sanctioned for a breach of the Rules of Competition, you should use this form.
